Liability Waiver & Release of Claims

I/The Student, the undersigned, understand that martial arts training is a physical activity involving risk of injury. These risks include, but are not limited to, bruises, scrapes, sprains, broken bones, and more serious injuries.

• Assumption of Risk: I/The Student voluntarily assume all risks associated with participation in martial arts classes, sparring, and events.

• Release of Liability: I/The Student hereby release UFMA, its instructors, staff, and owners from any and all claims, demands, or causes of action arising out of my participation in the program.

• Medical Treatment: I/The Student authorize the staff to seek medical treatment for the student in the event of an emergency if the emergency contact cannot be reached.

UFMA Code of Conduct

I (the Student, or the Parent/Guardian on behalf of the Student) agree to abide by the following standards:

• • Respect & Courtesy: I/The Student will show respect and courtesy to all instructors, staff, and fellow students at all times.

  • Hygiene & Equipment: I/The Student will maintain high standards of personal hygiene. All training kit and equipment must be kept clean, in good repair, and fit for purpose.

  • Ethical Use of Skills: I/The Student will use martial arts skills only for self-defence or within the authorised dojo environment. Under no circumstances will these skills be used for bullying, intimidation, or unprovoked aggression.

  • Punctuality & Attendance: I/The Student will arrive on time and ready to train. I will provide the club with advance notification of any planned absences or lateness.

Data Protection (GDPR) Consent

I (the Student, or the Parent/Guardian on behalf of the Student) understand and agree to the following:

• Data Collection: UFMA will collect and process personal data (including names, contact details, and any disclosed medical information) for the purposes of club administration, insurance, and student safety.

• Third Parties: We will not share your personal information with third parties for marketing purposes. Data may only be shared with relevant sports governing bodies or insurers where necessary for your participation.

• Communication: We may contact you via email, phone, or post regarding class schedules, grading, and club news.

• Your Rights: You have the right to request a copy of the data we hold, or to ask for its correction or deletion, by contacting the Club Secretary.

Media Waiver (Photo, Video, and Name Release)

I/The Student hereby grant UFMA permission to use the student’s name, likeness, image, and/or voice in any format (including but not limited to photography, video, and social media) for marketing, promotional, or educational purposes.

• I waive any right to royalties or compensation related to the use of these materials.

• I understand that these materials may be edited or cropped at the club's discretion.
